Request A Referral
In order to bill your insurance, I need a referral from your physician. Referrals can be faxed to my office at (360) 464-9999. If you would like to pay out-of-pocket without a referral, please contact me at 360-539-2988.
Verify Your Insurance Coverage
I accept many insurances, unfortunately, not all of them. It is your responsibility to make sure your insurance is included and that they cover our services (CPT code 97802 and 97803).

Check in For Your First Appointment
Virtual Appointment: You will be sent a link via email the day before your scheduled appointment. We use the platform Doxy.me which is HIPPA compliant and does not need an app installed on your device. Simply click on the link at your scheduled time using your device that has a camera and microphone available. Check in a few minutes early if you would like time to check your microphone and camera access.
Telephone Appointment: If you would prefer to consult over the phone we can accommodate you. Please let us know your preference at the time of scheduling your appointment.
